哺喂并发症的迹象
由神经系统缺损引起的常见哺喂并发症包括:
- 张力减退或“婴儿低肌张力综合征”,可能会出现或不出现肌无力症状,可导致无法有效控制口咽结构,进而导致吸吮力较弱或不协调
- 吸吮、吞咽和呕吐反射较弱
- 缺乏适当的哺喂所需机敏性和能量
- 吞咽困难,尤其是患有大脑性麻痹的婴儿
- 颈部和肩部过度伸展,影响了舌头定位和下颚运动
- 呼吸系统疾病,使得呼吸和吞咽更加困难
- 体重增加缓慢
- 增加母乳喂养并发症和生长发育缓慢风险
由唇裂和/或颚裂引起的常见哺喂并发症包括:
- 唇裂和/或腭裂宝宝往往无法在乳房周围形成密封。此外,由于在哺喂期间,口腔无法与鼻腔充分分离,因此宝宝不能形成真空,吸出乳房或奶瓶中的乳汁,或吸吮时非常困难
- 因此,这些宝宝在母乳喂养时会非常疲倦,母乳喂养时间较长,影响生长发育和营养摄入
- 宝宝唇裂和/或腭裂的尺寸和位置会影响其是否可以以及如何母乳喂养。研究表明,可在唇裂和腭裂手术之后开始或重新开始母乳喂养
母乳喂养评估

管理

- 如果母乳哺喂困难或不可能,或如果出现母婴分离,应在分娩之后尽早开始常规吸乳
- 建立和维持母乳供应量的方法:
